Expanding upon our 25+ years of experience providing physical, occupational, and speech therapy to Skilled Nursing Facilities (SNFs) and Life Planned Communities (CCRCs), Functional Pathways Preferred (FPP) was created in 2018 as our dedicated division for Senior Living Communities. This division has specialty programing to meet the residents’ needs on-site with the overarching goals for your community:

Pathways to Wellness is Functional Pathways’ solution to helping seniors thrive in place. The goals of the program are designed to improve the specific health and wellness needs of older adults based on the seven dimensions of Wellness.
Pathways to Wellness includes four specific Life Paths: Live Smart, Live Well, Live Connected, and Live Virtual. Each Life Path was developed to create opportunities for social interaction, mental exercise, and to enliven the spirit.
Depending on the resident population of your community, we will customize the Life Paths to create a package that meets the individual and collective Wellness needs of each customer.
From an independent study, it was found that 74% of residents said the Wellness Program was an important reason why they joined a community in the first place.
To ensure a seamless transition of care from one setting to the next, Functional Pathways can manage a client’s Home Health program or partner with their preferred Home Health provider. By utilizing Functional Pathways’ therapy staff, we can establish a continuum of care of specialty programs both on and off campus.
