To recap, only the top 10% of skilled nursing facilities within a state receive a 5-star rating on the 5-Star Quality Rating System, and two-thirds of Functional Pathways’ clients have earned a 4- or 5-Star rating, with over 80% achieving a 4- or 5-Star rating for Quality Measures.

The 5-Star Quality Rating System was created to compare nursing home quality based on health inspections, quality measures (QMs), and staffing. Health inspection ratings are dependent on the calculation of points extracted from results of nursing home surveys from the past three years, with recent surveys weighing more heavily in the calculation. Those results are used to rank nursing homes within their state, and the top 10% of those nursing homes get 5-star ratings.

Additionally, quality measure ratings are calculated based on a nursing home’s performance in long-stay ADL decline, mobility decline, catheter, high-risk pressure ulcers, physical restraints, urinary tract infections, moderate to severe pain, and short-stay pressure ulcers, and moderate to severe pain- delirium. “ADL decline” and “mobility decline” are ranked against other nursing homes in the state, and the other 8 quality measures are ranked against other nursing homes in the nation, with the top 10% of nursing homes receiving 5 stars.

Only the best facilities receive a 5-star rating, and only very successful facilities are even able to achieve a 4-star rating.

I cannot stress this enough because it only takes one person and one device to have catastrophic consequences.  Any device that does not have the latest updates is vulnerable to cyber security threats.  Hackers and criminals are banking on devices not having the latest security updates.  Security updates are the instructions on how to compromise a device and it’s data because they tell hackers where the system is vulnerable if it has not be updated.

Updates do not install automatically.  Many updates require a restart of the device.  If you see the update symbol in the taskbar of your laptop. DO NOT ignore it.  This means that the laptop has updates pending and needs to be restarted.   

I implore that everyone check and make sure all devices are updated. Below are instructions to check updates for your devices. 

Window Updates
Select the Start  button, and then go to Settings  > Update & Security   > Windows Update .

Go to Settings > General, then tap Software Update

Here is a quick Summary of what is going on.  A cybersecurity advisory went out yesterday for the healthcare and public health sector this weekend.  Federal agencies like the Cybersecurity and Infrastructure Security Agency (CISA), the Federal Bureau of Investigation (FBI), and the Department of Health and Human Services (HHS) have credible information suggesting an Eastern European threat group plans to launch a widespread Ryuk ransomware attack. 

As scary as this sounds and it is scary, these are three main avenues that attackers use. 

  1. Email phishing campaigns: The cyber criminal sends an email containing a malicious file or link, which deploys malware when clicked by a recipient. Criminals may also compromise a victim’s email account by using precursor malware, which enables the cyber criminal to use a victim’s email account to further spread the infection. -Why we say be careful when opening an email attachment especially when you are not expecting it.
  2. Remote Desktop Protocol vulnerabilities: RDP is a proprietary network protocol that allows individuals to control the resources and data of a computer over the internet. Cyber criminals have used both brute-force methods to obtain user credentials, and credentials purchased on darknet marketplaces to gain unauthorized RDP access to victim systems. Once they have RDP access, criminals can deploy a range of malware—including ransomware—to victim systems. -Why we stress using different and complex passwords for everything. 
  3. Software vulnerabilities: Cyber criminals can take advantage of security weaknesses in widely used software programs to gain control of victim systems and deploy ransomware. -Why we stress to always run updates and why there are always so many updates.

In order to protect ourselves please follow these best practices

  • Do not open any attachments in emails you are not expecting from anyone
    • Send a separate email to the sender to verify the legitimacy of the email
    • Contact support and forward the email to support@fprehab.com if you are not sure about an email before opening it.
  • Make sure to update your operating systems, software, and firmware as soon as manufacturers release updates
    • Do not ignore your windows, dells or iPados updates
    • Check once a week to see if you have updates that need to be ran, they do not install automatically and are often time ignored
  • Regularly change passwords to network systems and accounts and avoid reusing passwords for different accounts
  • Use multi-factor authentication where possible

Contact Tracing Scams

Many states use text messaging for their initial contact tracing outreach.  Scammers will mimic those text messages but they will contain a link to websites that request social security and/or insurance information as well as payments for testing.  Long story short, do not click on a link in a text message from an unknown sender.

One of the biggest questions right now is, “how do I clean my electronic device?” (Laptop, tablet, cell phone).  Cleaning an electronic device is actually pretty simple.  First, check the instructions that came with your device for directions on how to clean your device and what cleaners you should and should not use.

In general, the easiest way is to use a disinfectant wipe (Clorox, Lysol, etc.) to wipe down the device (see directions below from Lysol).  You can also use a disinfectant spray on a microfiber cloth to wipe down a device.  Remember to always spray the microfiber cloth and wipe down the device.  Never spray the device directly because the liquid has a greater chance of getting into the components of the device and damaging it.

Directions:

  • Smartphones and Tablets:  Use a Lysol wipe to disinfect the entire surface. Finish by wiping the screen with a dry cloth
  • Keyboards and Computer Mice: To clean your keyboard or mouse properly, first either turn off your computer or disconnect the keyboard and mouse from the computer. Next, use a vacuum with the correct attachment to remove all particles of food and dust. Then, use a Lysol wipe to wipe down the surface of your keyboard or mouse. Last, wrap a Lysol wipe around the tip of a Q-Tip or cuticle stick and circle the edges of each key to make sure that you reach every crack and crevice.
  • Gaming Consoles and Remotes: For actual game consoles, use a vacuum to remove all dust and wipe down the surface with a Lysol Disinfecting wipe. For remotes, first disconnect the remote or remove the batteries. Use a dry cloth to remove all dust and crumbs and then swab the entire surface with a Lysol wipe. Again, wrap your wipe around a Q-Tip or cuticle stick and make sure to get the edges around the buttons.

Did you get a new phone, watch, or iPad/tablet?  Did you bring it with you to work and want to connect it to the Wi-Fi, but no one is giving you the password?   Well, there is a reason for that.

Your device that connects to the internet is called an IOT (internet of things) device, which is any device that connects to the internet.  They range from toys, entertainment systems, watches, laptops, cells, tablets, and kitchen appliances to name a few.  Since these devices connect to the internet, they can also pose a security risk that many people are unaware of.

 

Here are some basic tips to keep you safe:

Do not connect to a public Wi-Fi

  • It is not secure and hackers can easily steal your information and see everything you are doing

 

Do not connect to a work network unless permission is granted or they have a separate Wi-Fi for employee personal devices

  • Just like with public WI-FI, work networks can see everything you do as well
  • Your device can bring malware and viruses that can spread over the network

 

Check for updates: You do not know how long the device has been in the box and how many security updates have come out since the device was made

  • Extremely important for cell phones, tablets, and laptops

Change the default password to access settings of the device

  • Main route for hackers to gain access to your network and everything connected to it
  • If connected to a work network, that can cause a catastrophic disaster

If switching to a new device, update, and back-up the old device before setting up the replacement device

Do not forget to “Factory Reset” any device before trading it in or disposing of it
