LONG-TERM CARE
Functional Pathways collaborates with the community’s interdisciplinary team, restorative nursing staff, and activities department to develop a comprehensive strategy to promote functional independence and quality of life. We have an extensive program that includes an ADL training module that can be utilized to educate staff on the proper identification of each resident’s functional abilities for ADLs according to Section G coding definitions.
Additionally, our Clinical Outcome Specialists survey the current residents’ needs to determine programming to enhance performance in Nursing Home Compare 5-Star Quality Measures (QM), SNF QM, Quality Reporting Program (QRP), and Value-Based Purchasing (VBP) Quality Measures.
